Add/Drop Form - To drop/add a class during the Drop/Add period (first 2 weeks of the semester)
Withdrawal Form - To withdraw from a class after the Drop/Add period (no later than 10th week of class)
Cross-Registration: GTU - To add a GTU course to the registration system
Cross-Registration: UC Berkeley - To enroll in a UC Berkeley course
Special Reading Course Form - To apply for a special reading course

Incomplete Petition Form - To petition for a course incomplete until all assignments are completed
Pathway Declaration/Change Form - To declare or change program pathway (part time/full time)
Selectives Completion Form - To count an elective toward Selective requirement
Graduate Substitution Form - Petition to substitute a PLTS or GTU course for a PLTS required course
Graduate Petition to Transfer Credit - Petition to transfer credits from another graduate program to count toward degree requirements (must submit course syllabi with form)
Graduate Petition General Purpose Form - To petition for late add/drop/withdrawal or extension of incomplete
Leave of Absence Form - File for a Leave of Absence
Separation Form - To notify PLTS of your decision to leave your program

Preferred/Chosen First Name Change Form - To change preferred first name in PLTS email, MyCLU Portal, and other internal systems
Name/Address Change Form - For legal name changes and address changes
Graduation Application Update Request Form - To update graduation information (diploma name, graduation term, diploma mailing address, permanent address)
